All students participating in interscholastic sports must have a current physical on file. The form is attached. Please download it, have it completed, and return it to school. In addition, students must also have a notarized Assumption of Risk form on file. Assumptions of Risk are valid for the entire time a child is enrolled at Wickford Middle School.
